No legal answer is the same as a moral answer here — both have to be argued. Choosing “Denunci la frode: la scienza deve restare integra, anche se i trattamenti vengono sospesi” prioritises the strict rule; choosing “Taci: interrompere cure efficaci causerebbe morti certe per un principio astratto” gives more weight to a context-aware exception.
